Types of acceptable proof of COVID-19 vaccination are as follows:

  • A photo of a physical vaccination card that includes front and back images
  • A digital verification obtained from myvaccinerecord.cdph.ca.gov
  • A digital verification that confirms your vaccination status from your assigned vaccination site

Guests 18 years of age or older will also need to bring a photo ID that matches the name on the vaccination card.

Acceptable forms of ID are:

  • A government issued ID
  • A driver’s license
  • A passport

Guests who are 12 -17 years old are also required to provide proof of vaccination, but there is no requirement to provide photo identification.
Guests under 12 years of age are not required to provide ID, proof of vaccination, or proof of negative COVID-19 test.

Vaccination accommodations will be considered on an individual basis for guests with a qualifying medical condition that prevents them from receiving the vaccination or who object to being vaccinated on the basis of sincerely held religious beliefs and practices. Individuals requesting a religious or medical accommodation will need to complete an attestation form upon arrival and, consistent with Los Angeles City Ordinance No. 187219, must also provide proof of a negative COVID-19 lab test (PCR or antigen) taken within 72 hours of entering our facilities.
